Comprehensive Geriatric Social Work Assessment + Custom Plan of Care
What is it?
A comprehensive geriatric social work assessment is an in-depth look at everything going on with an older adult in order to come up with a plan that will optimize their safety and outcomes.
By taking a Bio-Psycho-Social approach I'll look at what's going on physically, mentally and socially with your parent. The plan will also consider social and financial resources available.
Based on the assessment, I'll determine in what areas your parent may be at risk or need more support and make customized Plan of Care.
What Do I Get?
- 1 - 2 hour comprehensive assessment. This will be completed "virtually" over Zoom Video Conferencing.
- A written Plan of Care specific to your parent which will include recommendations for services, interventions, resources and referrals prioritized by risk.
- Preliminary research on the resources specifically available to the senior and/or in their community, as they apply to the suggested plan of care.
- The Plan of Care will be emailed to you in PDF format within 5 business days of the assessment.
